I ALWAYS LIKE to reflect on the past year and share some new things that I have learned, implemented, or changed.  First, I use the reciprocating handpiece or the Endo-Express® on all my cases.  NiTi is a thing of the past for me.  (I was formerly a big NiTi user!)  My results are just as good, and I have less stress during procedures.  The reciprocating handpiece prevents binding, and the reamers engage dentin only during the clockwise rotation.  I love the fact that on challenging cases I can pre-bend the reamers and place them in the canal and then attach the handpiece and clean and shape away.  I have no more fear of breakage and no more hand fatigue.  There is really no reason not to use the reciprocating handpiece.
   The Pleezer® from EDS is a tapered Peeso reamer.  This instrument creates a beautiful-looking tapered canal and eliminates the use of the # 2 Peeso and the # 2 Gates Glidden.  I use the Pleezer after I get my 20 reamer to length, and in a tight canal I may use it again before using my 30/.04.   The fact that we are using one instrument instead of two saves time and money.
   The 25/.06 NiTi reamer from EDS was introduced this year.  It is a wonderful shaping and cleaning instrument.  Sometimes the jump from the 30/.04 to the 25/.08 can be severe, and the 25/.06 creates a nice transition.  I am happy to get the 25/.06 to my working length loosely.  I can obturate with a Diadent 25/.06 point and EZ-Fill® and get a beautiful and successful result.
   We all know that things come up and we cannot always finish root canal therapy in one visit.  Rinsing with liquid EDTA and chlorhexidine before filling removes the smear layer from the dentinal tubules and has anti-bacterial effects.  I have improvised and started using calcium hydroxide powder mixed with chlorhexidine and an intra-canal medicament between visits.  I have had success with this slurry.

When doing your final rinse with chlorhexidine it’s important to leave it in the canal for two minutes. I also like to initially agitate it in the canal using my final SafeSiders instrument in the reciprocating handpiece. I then re-flush the canal and wait my two minutes. This procedure helps to insure movement of the solution to the apex. Overkill? Maybe, but like chicken soup for a cold . . . it won’t hurt!
